Contractor Information Required for Permit Issuance

  • State of Tennessee Contractor's License (or Home Improvement/LLE/LLP)
  • Certificate of Insurance with Workers Compensation/Exemption (in the same name as the above license)
  • Town of Smyrna Business Tax License OR Business License Release Form stating that the business license is not required at this time.
